- Vendor Background Questionnaire – Pages 35-41 - SIGNED AND NOTARIZED.
- Contract Signature Page – Page 85 and Pages 86 or 87 (whichever applies)
- Schedules B through E – Pages 88-92 – SIGNED AND NOTARIZED (where required)
- Certificate of Contractor Registration for prime contractor with the NYSDOL Public Work Contractor and Subcontractor Registry – failure to submit this Certificate with your bid will result in the disqualification of your bid.
- Wicks Subcontractor Forms - failure to submit these forms with your bid, in a separate sealed envelope, will result in the disqualification of your bid
