96541 - State of Utah Vision Plan

Public Employee’s Health Program (PEHP) is a non-profit trust that provides self funded medical, dental, life, accident, long term disability, and flexible spending accounts to public entities. We are seeking a vision plan to offer to the State of Utah employees, as well as State Risk Pool employees, who are eligible to participate with State benefits, our Local Government Risk Pool (LGRP) employees and retirees under our Medical Supplement plans. The contract will be through PEHP, not the groups we represent; as such the selected vendor will be working directly with PEHP for all payments, enrollment files, benefit fair invitations, marketing, education, and all communications.

In addition to a traditional vison plan, alternative ideas for vision coverage may be considered.
