Summary of Specifications:
1. Introduction This describes the terms and conditions to remove and replace tile flooring and wall base at the Public Services Compound, as listed herein. The project address is 350A South Marine Boulevard Jacksonville, NC 28540. The work shall include furnishing all safety equipment, materials, tools, labor, transportation, permits, and supervision necessary for the prosecution and completion of the work.
2. E-Verify North Carolina General Statutes, specifically §160A-20.1(b), prohibit the City from entering into contracts with contractors and subcontractors who have not complied with the requirement of Article 2 of Chapter 64. The Contractor shall submit the E-Verify Affidavit included in this Request for Quotes. Quotes that do not include this Affidavit will be considered non-responsive.
3. Quotes Due All quotes are due to Allen Baker by Tuesday, February 25, 2014 at 3:00 p.m. Interested bidders must sign this Request for Quotes and either fax it to (910) 938-5072, or deliver it to Allen Baker at the Public Services Division, Jacksonville City Hall, located at 350A South Marine Boulevard, Jacksonville, NC 28540.
4. Pre-Bid Meeting A Pre-Bid Meeting will be held on February 21, 2014 at 9:00 a.m. at the project address, 350A South Marine Boulevard, Jacksonville, NC 28540. This will allow potential bidders to verify all measurements and specifications. If you are unable to attend the meeting, and would like to schedule an appointment to come by the project location, please call Allen Baker at (910) 938-5328.
5. Insurance The successful bidder shall provide the City of Jacksonville Certificates of Insurance and endorsements with the following minimum levels of insurance coverage.
The Certificate of Insurance shall include:
• Comprehensive General Liability, $1,000,000 Single Limit, $2,000,000 Aggregate • Comprehensive Automobile Liability, $1,000,000 Single Limit • Workmen’s Compensation Insurance as required by North Carolina Law
The City of Jacksonville must be listed as an additional insured with respect to General Liability and Automobile Liability without exceptions. A valid certificate of insurance must be in place during the life of the contract. Additionally, insurance carriers must be licensed to do business in the state of North Carolina.
6. Qualifications The successful bidder must be properly licensed to do business in the City of Jacksonville. The City is exempt from Federal Excise Tax. Our Federal Tax Identification Number is 56-6000232. The City is not exempt from NC State & County Sales Tax. Such taxes shall be shown as a separate item on all invoices.
7. Rights The Jacksonville City Council reserves the right to waive any informalities, to reject any or all bids, and to accept that Bid or Bids which appear to be in the Owner's best interest.

9. Inspection All materials/supplies shall be the subject of inspection by the Project Manager at time of delivery. Permits and inspections will be the bidder’s responsibility
10. Quotes to be Retained Quotes are to remain valid for a period not to exceed 30 days.

12. Award of Bid The bid will be awarded to the lowest responsive, responsible bidder, taking into consideration quality, performance, and time.
13. Specifications • Remove and replace approximately 2400 sq ft of VCT tile and all wall base trim meeting tile • Submittals of product specifications, (Vinyl Composition Tile, Cove Wall Base, and All Adhesives) shall be attached to bid documents.
• Tile shall be installed according to manufacturer’s recommendations. • Mannington Essentials Commercial Vinyl Composition Tile in a standard color shall be .125” thick, 12” x 12” through- pattern construction with a minimum 5 year commercial warranty or equivalent. • Vinyl Composition Tile Adhesive, Taylor 500 Adhesive Encapsulater or equivalent. • Commercial Johnsonite Tightlock Rubber Wall Base 1/8” x 4 1/2”x 48” with APAC 750 Wall Base Adhesive or equivalent. • Debris removal will be the responsibility of the Contractor • Work area shall be kept clean and free of any safety hazards
