Request for Quotes
All quotes are due on 4/20/2016 by 12 p.m. for the service of repairing insulation around effluent pump motors at Land Treatment Site. Interested bidders must sign the attached Request for Quotes and either email it to bwilson@jacksonvillnc.gov or deliver it to Lines Maintenance Division located at 350 B South Marine Blvd, Jacksonville, NC 28540.
Qualifications
If not currently a vendor for the City of Jacksonville, the successful bidder must provide a Vendor Application and W-9 to the City of Jacksonville. The City is exempt from Federal Excise Tax. Our Federal Tax Identification Number is 56-6000232. The City is not exempt from NC State & County Tax. Such taxes shall be shown as a seperate item on all invoices.
Rights
The Jacksonville City Council reserves the right to waive any informalities, to reject any and all bids, and to accept that Bid or Bids which appear to be in the Owner's best interest.
Inspection
All materials/supplies shall be the subject of inspection by the Project Manager at time of delivery.

Quotes are to be remain valid through 5/20/2016.

Option A: Provide Supervision, labor and material to remove all existing fiberglass insulation and reinsulate new fiberglass with Aluminum Jacket at Waste Water Facility.
Option B: Provide Supervision, labor and material to patch existing fiberglass insulation, cover all old and new fiberglass with Aluminum Jacket at Waste Water Facility.
