By completing this form and placing a bid, your company agrees to meet the following terms and conditions in addition to the City of Jacksonville’s normal purchase order protocol for the Jacksonville Police Departments annual uniform purchase order contract for FY14. Please submit a bid what our agency will be charged after all fees and shipping cost per item for the next fiscal year 14. 1.You agree to stock specific quantities of all quoted items so that any order placed by the designee of the Jacksonville Police Department will be shipped within two weeks of your receipt of the requested order. The only exception to this is if a specific item is on back order through the Blauer Company. All Blauer back ordered items may be verified at any time. 2.You agree to designate and assign an in store sales representative to handle the Jacksonville Police Department’s account. This sales representative will be available Monday through Friday from 8:00AM to 5:00PM for all uniform requests, questions and exchanges. A mobile cell phone number will also be made available for after-hours emergency order requests and issues that may arise. 3.You agree to keep records which will reflect all invoices and billing by date for this account. Furthermore, it is the responsibility of your company to produce documentation to reflect invoicing and billing if requested by the designee of the Jacksonville Police Department. 4.All order requests must be filled with the new 2013/14 inventory from the Blauer Company. Any of these additional terms and conditions may be verified at any time during this fiscal year.

DISPUTE RESOLUTION: Any dispute arising from a forthcoming agreement to construct this project shall be subject to non-binding mediation in accordance with the rules as set forth in City of Jacksonville Resolution #2002 – 28 titled “Senate Bill Dispute Resolution”. The following disputes are not subject to mediation: i.) A dispute seeking a non-monetary recovery; and ii.) A dispute seeking a monetary recovery of $15,000 or less.
