PROPOSED SOLE SOURCE AWARD TO CATHOLIC COMMUNITY SERVICES OF ROCKLAND TO PROVIDE MULTI-SERVICE HUB SERVICES IN ROCKLAND COUNTY - $162,513
 
The County of Rockland is considering the issuance of a Sole Source award to Catholic Community Services of Rockland for the provision of Multi-Service HUB Services to residents of Rockland County. Funding amount is contingent on the adoption of the County of Rockland 2017 Annual Budget. As such, the final funding amount may differ from the estimated amount provided in this notice.
 
This award may be made on a sole source basis in accordance with the Rockland County Procurement Policy.
 
Program/Services

• Information and Referral
• Multi-lingual (English, Spanish, French-Creole) Case Management Services primarily in the areas of Haverstraw and Spring Valley including
  • comprehensive needs assessment to prioritize areas of most critical need as well as identify solutions for the long term
  • planning and assistance to individuals and/or families to address needs both short and long term
  • assisting clients with navigating issues including, but not limited to, budgeting, locating housing, navigating the public transportation system, and providing additional services to those individuals requiring specialized support due to frail health, mental health issues or other specific need
• Homelessness and Eviction Prevention
• Financial Literacy Services
• Nutrition information to the community
• Demonstrated successful experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met
• Provider must include a written attestation that it will be delivering more than one of the following services to all its clients, as applicable, under the resulting contract:

• Intake, case maintenance and management services* to help families achieve and sustain self-sufficiency
  • *for all federal and non-federal assistance programs; Medicaid, daycare; SNAP
• Child Preventive Services (referral, monitoring supportive and rehabilitative services provided to children under the age of 18 and their families to avert disruption of family unit)
• Child Welfare Services (services to protect and promote the welfare and safety of all children, including individuals with disabilities; homeless, dependent or neglected children)
• Family Preservation Services (services for children and families designed to prevent harm to children and help families at risk or in crisis)
• Family Support Services (community-based services to promote the well-being of children and families designed to increase the strength and stability of families and enhance child development)
• Staff Development Training and Educational programs (services to improve job performance and knowledge on social service programs)
• Refer and Inform Clients about the Home Energy Assistance Program (HEAP)
• For clients receiving employment services, Provider must include a written attestation that all of its clients through a contract with the Department of Social Services will be referred to the New York State's MyBenefits website, to ensure that all clients log on to complete an application and initial screening, informing clients about various employment supportive services and assistance programs, such as: Temporary Assistance for Needy Families (TANF); Federally Non-Participating Safety Net Employment Program (FNP); Supplemental Nutritional Assistance Employment and Training Program (SNAP E&T). 

• Multi-lingual (English, Spanish, French-Creole) Case Management Services primarily in the areas of Haverstraw and Spring Valley.  A minimum of 200 individuals and/or families must be served. 
   
• Information and Referral A minimum of 1,000 individuals and/or families must be served. 
   
• Homelessness and Eviction Prevention A minimum of 100 individuals and/or families must be served. 
   
• Financial Literacy Services A minimum of 70 individuals and/or families must be served.
   
• Nutrition information to the communityA minimum of 4 healthy cooking demonstrations will be given
   
• Demonstrated Successful Experience providing the above services to eligible individuals and families by records indicating Performance Target Outcomes have been met
