RFP-RC-04-036 - Providers to run Sustainable weight loss programs

Summary of Specifications:
The purpose of this RFP is to inform potential providers of an opportunity to provide public, sustainable weight loss programs to the people who live, work or attend school in Rockland County. As part of the Steps to a Healthier New York Campaign, the Rockland County Department of Health will award one-year grants to fund weight loss programs focusing on both sustainable weight loss and program-trainer training. Providers may be hospitals, health clinics, community agencies, faith based organizations, private medical groups, corporations, or individual health professionals who can meet the terms of this RFP. Weight loss programs must consist of a support group and an exercise component and should be targeted at overweight or obese adults, teenagers, and/or children. The program(s) should be community-focused, targeted at underserved populations, and easily accessible to community residents.
