Proposal is to contain this entire RFP, cover page through and including page 85, and the Signed Signature page. Submit one original and two (2) copies of the executed proposal in a sealed envelope. Additionally the proposal response must consist of one copy in electronic format on a CD, DVD or USB flash drive formatted in Adobe (.pdf) or Microsoft Word. The electronic copy that is an exact copy of the fully executed original may be submitted at the MITN site for this offering. See attached document for full specifications and performance requirements. Each bidder must indicate compliance with these specifications by marking "YES" or "NO" in the appropriate column for each individual paragraph of this specification.
Each bidder shall furnish satisfactory evidence of the manufacturer's ability to construct, supply service parts and technical assistance for the apparatus specified. Each bidder must state the location of the factory and location for post delivery service.
