Summary of Specifications:
1. The City of Warren Dental Insurance program will be marketed in accordance with specifications received. Exact duplication of benefits is a requirement of your bid. Should your proposal not meet or exceed these specifications, the agent shall highlight those areas in the actual proposal and clearly outline them in the “exceptions” section of the bid proposal forms. 2. Specimen (sample) contracts, summary plan descriptions, billings, ID cards, and any other material you believe to be helpful in our evaluation of this matter shall be submitted along with each proposal. 3. Agents shall prepare and submit a complete marketing report with all final quotations indicating the carriers approached, the proposals received, deviations form the current program, and in the case on declination the reason for declination.
