RFP 23-09-WP - Insurance Worksite Products

The Santa Rosa County School Board, Milton, Florida, solicits your company to submit a proposal on RFP 23-09-WP Insurance Worksite Products. 

This RFP solicits proposals for Insurance Worksite Products to include Cancer, Critical Illness, Hospital Indemnity, Accident, Long-Term and Short-Term Disability, Dental, Vision, Life and Voluntary Life.

Basic Life/AD&D Insurance:

• Current Benefit: 
  • Carrier:  Voya
  • All Full-time Active Employees participating in the Major Medical Program receive $50,000.  All Full-time Active Employees not participating in the Major Medical Program receive $100,000.  Retirees receive $10,000 with no AD&D.
• Requested Benefit Or Better: 
  • Match current.
  • Current Commission:  15%

• Group Life insurance should be portable and convertible.
• Provide monthly, quarterly, and annual claims reporting including participation data.
• Any in—force amounts for Retirees must be grandfathered.

• Current Benefit:
  • Carrier:  Voya
  • Employee:  $20,000 to a maximum of $300,000 in increments of $10,000
    • Guarantee Issue:  $200,000
• Requested Benefit or Better:
  • Employee:  $20,000 to a maximum of $300,000 in increments of $10,000

  • Spouse:  Increments of $5,000 up to a maximum of 50% of the employee’s election
