This RFP solicits proposals for Insurance Worksite Products to include Cancer, Critical Illness, Hospital Indemnity, Accident, Long-Term and Short-Term Disability, Dental, Vision, Life and Voluntary Life.

Basic Life/AD&D Insurance:

- All Full-time Active Employees participating in the Major Medical Program receive $50,000. All Full-time Active Employees not participating in the Major Medical Program receive $100,000. Retirees receive $10,000 with no AD&D.
- Requested Benefit Or Better:
- Match current.

- Group Life insurance should be portable and convertible.
- Provide monthly, quarterly, and annual claims reporting including participation data.
