Summary of Specifications:
PSD seeks to contract with a full-service disease management provider. PSD is requiring that the following disease states be included in the Disease Management Program: asthma, Chronic Obstructive Pulmonary Disease (COPD), congestive heart failure, coronary artery disease, diabetes, hypertension, and mood disorders. The Disease Management program must include population identification process, practice profiling, evidence based practice guidelines, collaborative practice models to include physician and support-service providers, patient self-management education, process and outcomes measure, evaluation and management, and routine reporting and feedback loops which may include communication and coordination with the patient, physician, health plan including the in-house EAS program, ancillary providers and the employer. The purpose of this program is to improve the quality, consistency and comprehensive care for those with chronic illnesses, thereby improving health outcomes, reducing morbidity, and reducing the use of high cost interventions when lower cost alternatives with equal efficacy are available.
