Additional Bidding Instructions

Submissions must include:
1. Attachment A – Offeror’s Certification & Addendum Acknowledgement
2. Attachment B – Bid Form/Specifications
3. Answers to the IFB Questions section
- Restate questions and provide an answer for each.
4. Attachment C – IT Technical Questionnaire
- This form is required by District’s Information Security department.
- Many questions on the form are specific to software. Complete the form questions that are relevant to products being bid as they relate to technical requirements and networking. For questions that are not relevant, leave answer spaces blank or mark “N/A”.
5. Attachment D – Consensus Assessment Initiative Questionnaire – CAIQ
- Software Security Assessment and Self Report
- An Excel workbook containing a standardized security assessment as required by the District’s IT Information Security Team.
6. Service Provider Identification Number (“SPIN”), or proof of SPIN application
7. State of Colorado Certificate of Good Standing or like form from the state in which company is located
8. Form W-9, W-8BEN-E, or W-8BEN
9. Any additional documentation vendor desires to submit (not to exceed a total of 5 pages)
