Summary of Specifications:
Colorado Springs Utilities (UTILITIES) seeks proposals for services effective January 1, 2013, to include: • Basic Life/AD&D (active employees) • Basic Life (retirees) • Voluntary Supplemental Life (active employees) • Voluntary Supplemental AD&D (active employees, spouses, children) • Voluntary Supplemental Dependent Life (spouses, children) • Voluntary Short-term Disability Insurance (quotes should include fully-insured and self-insured funding options) (active employees) • Voluntary Long-term Disability Insurance (active employees) In addition, you may include quotes on other optional services, such as voluntary benefits or integrated benefits that complement services listed above (i.e. integrated EAP services in disability programs). Resulting contracts shall be a Two (2) year agreement with Three (3) - One (1) year renewal options to be exercised at the option of UTILITIES
