In order to be considered responsive, vendors must submit all of the following with their bid submission:

Solicitation and Response Cover Sheet
IFB Price Submission form with pricing for each plant location (Attachment D)
Product specifications for bulk cold asphalt patching material.
Current asphalt cement (A/C) pricing as a benchmark for future renewal price increases.
