In general, the successful proposer will provide a Performance Coach/Wellness Coordinator for the City’s Human Resource Department located at 301 W. B Street, Pueblo, CO 81003. As needed, the Coach/Coordinator may be required to assist at other locations and departments located within Pueblo County. It is the intent of the City to award the project for the remainder of 2018 with possible annual renewal based upon the City obtaining adequate wellness program funding. The City anticipates starting this wellness program as soon as possible after award, no later than September 1, 2018, with possible annual extensions through December 31, 2021. All interested service providers shall meet all requirements as listed within the RFP.
