INTENT
This quote intends to establish a pricing agreement for on-site hospice care services provided to incarcerated patients. Maricopa County, through its Department of Correctional Health Services (CHS), facilitates compassionate end-of-life care within the Maricopa County Jails, ensuring dignity and support for patients in need at the Lower Buckeye Jail Infirmary located at 3250 West Lower Buckeye Road, Phoenix, Arizona 85009.

The County reserves the right to award in whole or in part, by item or group of items, by section or geographic area, or make multiple awards where such action serves the County’s best interest.
