Funding for the HUB Network Investigation grant and HUB Network Activation grant comes from a five-year CDC-funded cooperative agreement called “DP18-1817: Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes and Heart Disease and Stroke.” This five-year grant runs from September 30, 2018 through September 29, 2023. The purpose of the cooperative agreement is to “support the design, testing, and evaluation of novel approaches to address evidence-based strategies aimed at reducing risks, complications, and barriers to prevention and control of diabetes and cardiovascular (CV) disease in high-burden populations.” The expansion of the HUB Model and Network is also an important strategy within the grant enhancing opportunities for health equity. Through time, all partners in the Wisconsin HUB Network will address desired heart health and diabetes prevention and control objectives as required by the funding source.
