Specifications include, but are not limited to: Develop two short, written reports: Mid-project update (approximately 1-2 pages) by January 31, 2021. This update should document progress toward meeting objectives of the work plan and any barriers the grantee is encountering. Final report (approximately 2-3 pages) by August 14, 2021. This final report should include results and outcomes of implementation of the work plan, lessons learned, and barriers. Grantees may also choose one of the following two options in lieu of a written final report, as long as the chosen activity takes place during the grant period (July 1, 2020 to June 30, 2021) and the format can be shared with the Minority Health Program by the same deadline of August 14, 2021. Disseminate relevant results and outcomes that come from implementation of the project (e.g., via listserv, newsletter, presentations at conferences, journal articles, social and earned media, webinars). Present on the project at a community meeting, conference, or other event (e.g., Wisconsin Public Health Association conference, Wisconsin Tracking’s Technical Advisory Group, Wisconsin Association of Local Health Departments and Boards (WALHDAB) meeting, webinar). Coordinate with Minority Health Program staff to schedule a site visit sometime before the end of the second quarter (December 31, 2020).
