3.3. PROGRAM DESCRIPTION This program will provide outreach and referral services for persons ages 18 and over who identify as lesbian, gay, bisexual, transgender, queer (LGBTQ+). Referral services may include but are not limited to referrals to community supports, housing and food security resources, and health care. Outreach activities may include provision of educational materials, workshops, targeted outreach, drop-in groups, and other community supports. 3.4. GOALS A. Increase service access and supports for Dane County residents who identify as LGBTQ+ through the provision of information, referral, and outreach services. B. Ensure dissemination of accurate information about available services and the needs of Dane County’s LGBTQ+ population. 3.5. NEEDS AND EXPECTATIONS A. The program must provide multiple points of access, including phone, text, online and in-person walk-in services for referrals, information and outreach activities. B.
