Provide mentoring services to at-risk and system-involved youth. Youth Mentoring includes services that will provide: (1) individual (one-on-one) supports, (2) group support, (3) academic/vocational assistance as needed, and (4) pro-social enrichment activities. 3.4. GOALS A. 85% of youth will see improved school attendance/participation B. 80% of youth will reduce at-risk behaviors C. 85% of youth referred will successfully participate and complete programming D. Complete monthly 711 reports, quarterly narrative reports and monthly progress reports. These reports will include demographic information, units of service, number of new clients as well as program progress. 3.5. NEEDS AND EXPECTATIONS A. Provide services to 12 youth annually with 6 youth served at any point in time. Youth may be referred by DCDHS, schools or self report. B.
