Specifications include, but are not limited to: The Wisconsin Department of Health Services (DHS) is seeking bids for Wisconsin National Core Indicators (NCI) Surveys. This includes both the National Core Indicators In Person Survey (NCI IPS) and the National Core Indicators – Aging and Disabilities (NCI-AD). DHS intends to use the results of this process to award a Contract. National Core Indicators is a national project that includes a comprehensive suite of surveys and support for accurate administration, provided to participating National Association of State Directors of Developmental Disabilities Services (NASDDDS) member agencies, to gather a standard set of performance and outcome measures that can be used to track their own performance over time, to compare results across states, and to establish national benchmarks. It includes a series of survey tools for families, individuals who receive services and a Staff Stability Survey. The National Core IndicatorsAging and Disabilities (NCI-AD™) is a similar initiative designed to support states’ interest in assessing the performance of their programs and delivery systems in order to improve services for older adults and individuals with physical disabilities. The NCI IPS for people with Intellectual and Developmental Disabilities (I/DD) is a face-to-face conversation carried out in person with individuals who are 18 years of age or older and are receiving at least one paid service from the State in addition to case management. It consists of Pre-survey Information used to facilitate the conversation and successfully schedule the survey; Background Information which is collected from records, case managers or other administrative sources which includes data about demographics, personal characteristics, health data and data on employment status and wages at minimum; Section I can only be answered by the individual receiving support. No proxy responses are permitted. Section II may be completed by a proxy if the interviewer determines the individual receiving services cannot answer for him or herself, a COVID supplement, and up to ten state specific questions provided by the State
