This program is intended to prevent the escalation of youth and family behavioral health needs to improve youth functioning in the school, home, and community through assessment, case management, service referral, and advocacy and consultation with the school team. This program serves nine school districts throughout Dane County through trauma-informed, wraparound case management. Currently operating as Building Bridges, this program promotes youth and family stability and wellbeing through assessment, crisis stabilization, case management, service referral, and advocacy and consultation with the school team. This program will serve students and their families who have been identified by participating school districts as likely needing behavioral health services and/or supports, and who require additional assistance to access these services.
