The State of Wisconsin, Department of Health Services (DHS), is seeking information to inform Wisconsin’s application for the new Rural Health Transformation Program (RHTP). Wisconsin’s application for the federal grant opportunity must align with the required use categories and eligibility criteria defined by the One Big Beautiful Bill Act (OBBBA) and the Centers for Medicare and Medicaid Services (CMS). DHS intends to pursue a comprehensive approach to meet the needs of Wisconsin communities. We invite the submission of ideas, insights, considerations, and project concepts aligned with the goals of the RHTP. On July 4, 2025, the One Big Beautiful Bill Act (H.R. 1) was signed into law, establishing a $50 billion federal Rural Health Transformation Program (RHTP) to improve rural health care across all fifty states. At the federal level, the RHTP will be administered through CMS. In Wisconsin, allocated funds are expected to flow through DHS to support health system transformation. Through this Request for Information (RFI), DHS is seeking input to help shape Wisconsin’s application for the RHTP. The Department is committed to strengthening the continuum of care for rural residents, ensuring everyone in Wisconsin can access affordable, quality care where and when they need it. This information may be used to shape Wisconsin’s RHTP application, inform future funding opportunities and grant program design, identify high-potential projects for expedited development, and guide state and federal efforts related to rural health funding.
