Organizations receiving this award will be expected, at a minimum, to perform the following activities: Provide statewide health benefits counseling, coordination, and support for children and youth with special health care needs, and their families, to relieve unnecessary financial burdens and prevent loss of coverage. Design, create, and provide community and professional education and trainings on health financing changes/updates/requirements. Ensure CYSHCN programs, or others identified by the department, are trained to maintain access to services even when household circumstances change. Advocate with state leaders to make system changes to better serve CYSHCN and their families. Collaborate with state partners to identify challenges within Medicaid health benefits and strategies to improve state systems for families. Identify opportunities for updates to health financing, necessary education, and outreach for sharing information. Design and implement a knowledge assessment to be used annually to determine knowledge base of Children’s Resource Centers (CRC) guides and identify a yearly training plan in partnership with the state program. Implement yearly training plan and create and provide education and technical assistance to the CRCs and other CYSHCN programs. Ability to collect ongoing data regularly to demonstrate and analyze the impact of activities.
