Specifications include, but are not limited to: The purpose of this Request for Simplified Bid (SB) is to provide interested parties with information to prepare and submit a bid for providing Non-Emergency Medical Transportation Services for the resident(s) of Central Wisconsin Center (CWC) who require the use of a wheelchair for mobility. CWC is a resource center for residents with intellectual/developmental disabilities, located at 317 Knutson Drive, Madison, WI 53704. CWC is operated by the Division of Care and Treatment Services (DCTS) under the auspices of the Department of Health Services (DHS). CWC has a resident population of approximately 170 individuals. The Supplier will pick up residents at CWC or medical clinics for transportation in a single or round trip. These services are required on a year-round basis Monday thru Friday 6:00 AM to 4:00 PM. CWC estimates 240 trips per year using a wheelchair van transporter. Most trips will be round trips from CWC to medical appointments, however one-way trips may be required. CWC residents have medical appointments at 148 locations listed on Attachment E- Locations (subject to change). Locations will vary from day to day within a 35 mile radius of Madison but periodically some longer trips will be required
