Specifications include, but are not limited to: Vendor must have a minimum of ten (10) years’ experience. Vendor must have experience writing approved plans of correction, implementation of plans of correction, care plans, writing menus for long-term care facility with a population age ranging from 18 to 100+ and writing diets for patients with chronic and terminal illnesses Vendor must have knowledge of Federal and West Virginia regulations and experience with Federal and West Virginia survey processes Vendor must provide services during normal business hours of 8:00AM to 4:00 PM Monday through Friday (excluding holidays). Vendor will work a maximum of 16 hours per month and not to exceed 192 hours per year. Vendor will bill John Machin Sr Health Care Center a per hour rate for all services Vendor must have a valid WV Dietician’s license
