Specifications include, but are not limited to: 4.1.1 Vendor will provide in-house occupational therapy services for the residents of Lakin Hospital. Vendor must include the following services: 4.1.1.1 Vendor must complete evaluation on all residents and all new admissions. 4.1.1.2 Vendor must be available for consultations at the request of the agency. 4.1.1.3 Vendor must document each resident visit and maintain medical and statistical records in accordance with Lakin Hospital policies and procedures. 4.1.1.4 Vendor shall reassess resident upon consultation request and modify treatment plans all in coordination with the attending physician and appropriate treatment team members. 4.1.1.5 Vendor shall develop treatment/care plan for those residents on treatment plan. And re-evaluate residents quarterly and annually for modifications of treatment plans. 4.1.1.6 Vendor shall provide recommendations for treatment programs to improve ones’ ability to perform daily activities. 4.1.1.7 Vendor shall provide recommendations for adaptive equipment and usage training and performance skills assessments and treatments. 4.1.1.8 Vendor shall provide guidance such as explanation of treatments and explanation of any adaptive equipment to the caregivers. The occupational therapist would ensure personnel are knowledgeable as deemed necessary by facility requirements and residents’ needs. 4.1.1.9 Vendor will participate as necessary in appropriate committees and the indirect supervision (instruction and direction of procedures concerning therapy care) of restorative therapy staff. 4.1.1.10 Hours of service will “Not Exceed” 275 hours in a twelve-month period. 4.1.1.11 Vendor will work during normal business hours which is Monday – Friday from 8:00 a.m. to 4:00 p.m. excluding major holidays (New Year’s Day, Memorial Day, July 4th, Labor Day, Thanksgiving Day, the Friday after Thanksgiving and Christmas Day).
