The contractor shall provide the agency with Equipment and Systems Maintenance and Repairs on an open-ended and continuing basis as outlined in this contract. 5.1 Equipment and Systems Maintenance and Repairs 5.1.1 The Contractor shall perform Preventative Maintenance on a monthly basis, totaling twelve (12) visits per calendar year. Each visit shall be billed individually. Any additional charges for labor or materials resulting from corrective maintenance identified and performed during a Preventative Maintenance visit shall be itemized separately and must align with the pricing set forth in Exhibit A - Elevator Maintenance and Repairs Contract Pricing. 5.1.2 The Contractor shall perform Corrective Maintenance on an as-needed basis to restore the Elevator Systems to full operational condition. Labor for Corrective Maintenance shall be billed at the hourly rates specified, and parts shall be billed using the applicable pricing multiplier provided in Exhibit A - Elevator Maintenance and Repairs Contract Pricing. 5.1.3 The contractor shall respond to corrective maintenance requirements by telephone or in person within two (2) hours and must arrive onsite as soon as possible, but Page 4 of 12 REQUEST FOR QUOTATION ARFQ 0612 SP15260000011 no later than twenty-four (24) hours after the contractor is notified by the agency. No exceptions to the twenty-four (24) hour requirement unless granted by the agency. 5.1.4 Corrective Maintenance must be performed between the hours of 8:00 a.m. Eastern standard time (EST.) and 4:00 p.m. EST., Monday through Friday, excluding Holidays, unless the agency approves work at another time. 5.1.5 Agency may request Corrective Maintenance on an emergency basis by notifying the contractor of the emergency. The contractor must respond to all emergency callouts. 5.1.6 If the location has a building automation system (BAS) that controls the facility equipment, it is also part of this contract. 5.1.7 In this contract, at the location's discretion, they can purchase new equipment, devices, and parts by using the percentage markup provided by the contractor on Exhibit A, Pricing Page(s). 5.1.8 According to the location's discretion, they may have the contractor install new equipment, devices, and parts by using the corrective maintenance hourly labor rates provided by the contractor on Exhibit A, Pricing Page(s). 5.1.9 If the location has maintenance staff that can do corrective maintenance work on the equipment and systems, they are permitted to do the work. 5.1.10 The following information under this item will be for corrective maintenance, replacing parts, and installation of new devices and equipment: 5.1.11 The contractor shall quote an hourly rate for regular labor hours. Regular labor rate hours shall be between the hours of 8:00 a.m. and 5:00 p.m. EST, Monday through Friday.
