Specifications include, but are not limited to: A. Monthly Performance Reports, which will include documentation of: 1. Credible Messenger Reporting a. Mentorship training conducted as well as any program developments or enhancements. b. Number of time Credible Messengers meet with individual (1:1) clients. Note: Credible Messengers will be required to meet clients a minimum of four (4) times per month, documented; unless they attend groups, then two (2) times per month. c. Documented incentives administered to individual clients. d. Monthly Case Plan status, to include achievements, barriers, changes and case notes from contacts. 2. Client Reporting – Each youth will be documented individually along with specific outcome(s). Additional metrics will be developed in collaboration with JPCs (Case Plan). a. Number of young people referred into services, including date of referral. b. Age, ethnic and gender status of young people referred.
