Specifications include, but are not limited to: An effective N/S program would: Improve quality of life for those experiencing a TBI by assisting the connection with the person to the resources needed (private/public/local). The key would be a warm hand-off for case coordination. Examples include application assistance, system(s) understanding, and appointment setting. Partnership development in the TBI community through assessment of information/service gaps while conducting N/S services. Continually monitor N/S services to ensure AIRS standards and accountability structure is effective and meeting the needs of the person with TBI. Navigation and Support Services (N/S) engage in a more detailed eligibility assessment and/or determination for additional short-term assistance in navigation and support beyond the I&R transactions. Such as prompting inquirers regarding their interest in additional resources, conducting initial coordination, application assistance and appointment setting. Conduct a Client Case Conference with I&R staff, pre-screen client’s referrals for an occurrence of a TBI, verify and/or establish public - private - local resource eligibility and those currently in use. Determine the persons interest in N/S short term assistance and types of assistance required. A person with a screened TBI and/or their support network can and has the ability to participate in short-term N/S planning and development in forming and completing Person Centered Practices (PCP) goals. The Client can use a support network (if required) to assist with PCP goal forming and assistance in the completion for those agreed to short-term goals on behalf of the Client as needed. A signed copy of interest and agreement of goals will be documented and filed in the Client record, prior to starting N/S Services. Determine persons current needs and resources in family / support network, education, work, health, financial, and leisure. Define through an assessment for needs and develop a clearly defined plan for Client and N/S towards accomplishing those additional steps that N/S offers in a short period of time. Empowering the individual toward accomplishment and independent living. N/S does not fill the role of a Clinician and will not be present during appointments. N/S will provide self-advocacy techniques to the Client to advocate for themselves at the appointment. Work with agencies as needed to conduct a warm-hand-off for those Clients needing extra N/S with public - private - local resource programs and system. Client Case Note records will reflect all entries for all forms of communications in a chronological order: activity completed, contact with client or other organizations, and all collateral work done on the Client behalf, types and organizations of information and referral / resources supplied, mailing of materials / documents, and client meetings. Include documentation of all contact and updates.
