The County is seeking to identify qualified non-profit/not for profit providers interested in utilizing this facility under a low- or no-cost lease agreement to deliver medical respite services to homeless households. Medical respite services provide short-term medical support for individuals experiencing homelessness who are too sick or fragile to recover on the streets or in shelters, but do not require hospitalization. For the congregate setting as envisioned for this program, it would require 24/7 nonmedical staffing onsite, and either onsite nursing services or transportation to offsite nursing services. In addition, each person receiving services at the facility must: • Have 24-hour access to a bed, ensuring consistent and uninterrupted shelter, • Be provided three meals per day, meeting the nutritional needs to support their overall health, • Be provided transportation to all medical appointments, facilitating access to essential medical care and treatment, • Have access to a phone for telehealth and/or communications related to medical needs, ensuring continuity of care and communication with healthcare providers, • Be provided a safe space to store personal items, allowing them to securely manage their belongings during their stay, • Receive wellness checks at least once every 24 hours by medical respite staff (clinical or nonclinical), ensuring the ongoing monitoring of their health and wellbeing, and, • Be provided additional supportive services to assist them in care coordination and housing supportive services. These services could be provided by a partner agency with experience delivering such supports to the target population.
