Specifications include, but are not limited to: The purpose of this RFP is to solicit qualified contractors to provide a Residential Settings Locator for the Aging and Long-Term Services Administration (ALTSA). The application will match individuals in need of long-term care with potential Adult Family Home (AFH) and Assisted Living Facility (ALF) providers who can meet those needs and have vacancies available. The aim of the search application is to enable case management staff to efficiently find a qualified residential provider who can meet the client’s care needs. The Contractor shall provide the services and staff, and otherwise do all that is necessary to perform the work outlined below. a. Application implementation. (1) The Contractor shall provide existing Residential Settings Locator application with core functions and offering flexibility to meet business needs. (2) The application will facilitate Medicaid Providers in reviewing information added by ALTSA case managers. Information specific to client’s care needs and attached documents: (a) Assessment Details and Service Summary (3) Core functions of the application include: (a) Electronic file sharing (b) Single-sign-on authentication (c) Accommodation of specialized needs identified by ALTSA (4) Data Tracking and Reporting (a) Accurately track and report on active and non-active providers, new registered providers, and provide regular schedule reports. (b) Track and report data related to referrals, transition, withdrawals, and open referrals. (c) Reports will relate to serve optimization/capacity and processing issues that impact user work/workflow, including peak time usage patterns. (5) The application must fully comply with HIPAA as noted in the contract and is designated for ALTSA case managers to match Medicaid providers for clients ready to transition.
