Specifications include, but are not limited to: Eligibility and Screening 1. To establish eligibility for Kinship Navigator services, the Contractor shall screen kinship caregiving families to ensure a kinship caregiver is: a. A caregiver of any age who is living with and has primary responsibility for raising a child eighteen (18) years of age or younger, and b. Related by blood, marriage, or adoption. 2. The kinship caregiver may have formal or informal custody of the child(ren) and may or may not be currently involved with Department of Children, Youth and Families services. 3. Verification of Primary Caregiving Relationship: The Contractor must request the kinship caregiver provide current (produced within the last 12 months) verification of their primary caregiving relationship to the child(ren) they are raising, from a reliable source. a. If the caregiver has custody paperwork dated more than 12 months from their request for assistance, another current form of documentation must be provided. Reporting Requirements 1. The Contractor shall complete a Kinship Navigator Program “Screening/Contact Form” for each new caregiver accessing the services. 2. The Older American Act Performance System and Data Recording forms will be provided and must be used for recording services. 3. In addition to monthly submission of a Screening/Contact Form for caregivers, the following information, service data, and billing detail shall be reported and submitted monthly to the County: • Caregivers first and last name, and date of birth; • Child(ren) first and last name, and date of birth; • Caregiver address and telephone number; • Identify relationship between caregiver and child(ren) as by blood or marriage; • Caregiver demographics to include ethnicity, race, household composition, sex/gender, income at or below 100% of the Federal Poverty Level, Veteran status, language and English fluency. • Number of access and assistance case coordination service units time (do not include travel), per family, during the reporting month. • Total number of contacts, per family. • Any additional required data elements based on program requirements. The required Kinship Navigator Program data will be entered by Division of Aging and Long Term Care staff into the Community Living Connections (CLC) reporting system. 4. Annually, and no later than September 1st of each year, the Contractor is required to submit the following information to the Division of Aging and Long Term Care: At least two case examples describing the circumstances and the needs of a participating caregiver, what assistance was provided to the caregiver through the program, and the impact the intervention had on the kinship caregiver and their family to help or continue stability for the children in their care. 5. The Contractor is responsible for conducting and reporting results of an annual client satisfaction survey, and for participating in any statewide evaluation efforts mandated by Washington State Department of Social and Health Services.
