Specifications include, but are not limited to: A. Provide masters level clinical staff (Licensed Mental/Behavioral/Chemical Dependency Health Providers) for onsite mental and behavioral health services and/or chemical dependency (MH/BH/CDC) for students who are Medicaid eligible, private pay, or covered by health insurance. B. Provide MH/BH/CDC health treatment in individual, group, and family modalities for students and their families within the school-based setting. 1. Please list any specialty areas you/your organization can address. (e.g. abuse, neglect, sexual abuse, eating disorders, chemical dependency, etc.) 2. Please list the ages you/your organization can serve (you may choose more than one): under 6 years old, 6-10 years old, 10-14 years old, adolescent and adult. 3. Please list any restrictions you/your organization may have (i.e. licensure only for chemical dependency) C. Screen, evaluate and provide intake for students and families to determine eligibility. 1. Please describe your process for school personnel to refer students to MH/BH/CDC services. 2. Please describe your intake process for students from school referral through on-site service. D. Please list the languages spoken by you/your organization. If none other than English, are translation services available for counseling services and print materials? E. Attend meetings with school personnel, as needed, to provide appropriate services for students.
