Specifications include, but are not limited to: Basic Support Services (Level 1-3) Developing a Caregiver Support plan with a caregiver for each youth placed into their care. Helping the caregiver to monitor the child’s progress relative to the child’s service plan. Assist in coordinating the child’s referral to services. Providing case aide supports. Connecting the caregiver to community resources when appropriate. Assistance with transition to adulthood planning. Assist with on-call support service that include twenty-four/seven (24/7) crisis phone support and coaching. Enhanced Support Service (Level 4-7) Assist caregiver with school related supports. Assist caregiver with coordination with Apple Health Coordinated Care health services. Assistance identifying and accessing childcare. Parenting skills as requested on or off-site. Assistance to the caregiver in providing culturally relevant services to each child and assist the caregiver in maintaining the connection between the child and his or her culture. Support and assistance with licensing/CPS referral. Assist with on-call support services twenty-four/seven (24/7) and any development of a crisis plan that includes after-hours availability. Assistance with locating and scheduling respite care for each youth. Assistance with creating and implementing supervision plans as necessary for youth and caregivers. Assistance with coordinating case aide supports as needed. Case Aide Services Foster Care Child Support Aide Services, commonly referred to as Case Aide Services, are intended to provide trauma informed direct supervision that augments the supervision and activity plans for children whose behaviors or developmental needs cannot be managed in a foster home or family setting without additional assistance.
