Specifications include, but are not limited to: The Department of Social and Health Services (DSHS) contemplates awarding a sole source contract to PointClickCare Technologies Inc to provide access for Home and Community Services Division and Area Agencies on Aging Case Managers to a care coordination system. DSHS requires a Contractor to allow access to this robust care coordination system that has a robust cross system network of users and can facilitate information sharing, assist with care coordination, enhance case management functions, assist with bed holds and knowledge of admission, transfers, and discharges. PointClickCare Technologies Inc is uniquely qualified to provide access to a platform: that provides real-time notifications of an admit, discharge or transfer from an acute or post-acute setting. These notifications will fill gaps for CMs and streamline the process for timely and accurate service authorization. CMs could also use these notifications to identify clients who need a significant change CARE assessment due to increased care needs following a hospitalization or skilled nursing facility stay that receives prescription monitoring program data from Department of Health that is used by all hospitals across WA state that is utilized by all Medicaid and DSNP managed care payers in WA state that has a proven effectiveness when utilized by AAAs for care coordination of their Health Homes population that offers an Artificial Intelligence readmission risk score based on an individual’s recent medical history.
