Specifications include, but are not limited to: The Physician agrees to provide the following services in accordance with currently accepted professional standards and applicable federal, state and local law regulations. 1. ROUTINE (within the facility): Offeror must provide their own portable equipment if needed to perform the following required services: 1.1 Evaluate and provide preventative care for residents at risk of developing foot problems, such as diabetic residents and residents with circulatory disorders. 1.2 Must examine each resident every three to four months for routine foot care services. 1.3 Make appropriate referrals for necessary continuing foot care. 1.4 Make provisional recommendations for treatment, specifying conditions, and for medications, specifying type(s) of medication(s) and dosage(s). 1.5 Prescribe medication(s) only when necessary and only when approved by the SBVCC Medical Director and/or the residents attending physician. SBVCC will provide any prescription medication necessary from the SBVCC’s central podiatry. 1.6 The Podiatrist shall conduct podiatric clinics at SBVCC at a mutually agreeable frequency and schedule. 2. NON-ROUTINE (outside the facility): 2.1 Residents may be transported from SBVCC to the physician’s office or another source provider for special services on an as needed basis and only after written prior pre-authorization from the Medical Director, or designee. 3. DOCUMENTATION The Podiatrist shall document all services rendered in the residents’ clinical records in accordance with SBVCC policies and procedures.
