Specifications include, but are not limited to: Evaluations should follow the prescribed format as outlined in Attachment A, and utilize the standardized tests listed and described in Attachment B. All testing instruments utilized for psychological and capacity evaluations shall be objective or performance-based unless it has been determined by the referring party and the Contractor that there is clear and obvious incapacity such as in the late stages of Alzheimer’s disease. The evaluation shall contain an informed consent section in which the Contractor describes the purpose of the evaluation, the client’s understanding of the evaluation and its risks and benefits, as well as the client’s consent to participate in the evaluation. Psychological Evaluations of individuals referred by Project Officers shall be used by Arlington County Department of Human Services, Aging and Disability Services and Behavioral Health Divisions for diagnostic purposes, to inform treatment, and to determine eligibility for developmental disability services. These evaluations shall include: Scaled measures of adaptive functioning, preferably using the ‘Vineland Adaptive Behavior Scales’ Edition II or III; Assessing Intelligence Quotient (I.Q.) or range if unable to determine precise IQ, to determine intellectual functioning and capacity. Diagnosis of a developmental disability, which may include an intellectual disability (including severity thereof, e.g. mild, moderate, severe, or profound) as defined by the most recent amendments to the Virginia Code (https://law.lis.virginia.gov/vacode/title37.2/chapter1/section37.2-100/); Assessment of functional limitations in adaptive skills, including, but not limited to: receptive and expressive language, learning, self-care, self-direction, home living, social skills, health and safety, and work or capacity for independent living. For referrals seeking to establish or rule out an intellectual disability, the Contractor shall utilize a standardized cognitive testing instrument unless the Contractor determines that the client is unable to engage in such testing. In the event the client is unable to be engaged in cognitive testing, the Contractor shall state the reason in the evaluation and offer an assessment of intellectual functioning based on the clinical interview and/or other administered testing.
