Contractor shall abandon two (2) private drinking water wells located at 862 Stewarts Road in Lanexa, VA in accordance with the Virginia Department of Health (VDH) regulations and procedures. All work and/or goods provided shall, at a minimum, meet industry standards and state guidelines. 2.2. General Requirements A. The Contractor shall be licensed and experienced in well abandonment procedures. B. All work shall comply with Virginia Department of Health (VDH) standards and regulations regarding well abandonment. C. Work shall be performed in accordance with the American Water Works Association (AWWA) standards for well abandonment (AWWA C651). 2.3. Project Site and Existing Conditions A. The Contractor shall conduct a site assessment prior to commencing work to identify all well locations and conditions. B. The Contractor shall submit a site assessment report with recommendations on the methods of abandonment. C. Existing wells to be abandoned include: o Well #1: In woods edge on west side of property o Well #2: In open area near old house site o Map of well locations located in Section 8, Attachments 2.4. Equipment and Materials A. The Contractor shall be responsible for providing all necessary equipment, tools, and materials to complete the well abandonment. B. Materials used for sealing the wells shall adhere to applicable VDH standards, including but not limited to: o Concrete or bentonite grout. o The decontamination of all equipment and materials prior to their use at the site. 2.5. Well Abandonment Process Contractor shall: A. Remove casing and any surface debris as needed. B. Clean the well to remove any remaining water or sediment. C. Apply sealing material (grout) from the bottom of the well upward, as per VDH regulations. D. Ensure proper monitoring of the grout placement to eliminate voids. E. Complete Commonwealth of Virginia Uniform Water Well Completion Report (form GW-5). Sample form located in Section 8, Attachments. F. Perform final site restoration after abandonment is completed.
