Specifications include, but are not limited to: A. Contractorshall perform preventive and correctivemaintenance on Tax’s Spectrum Model 6040 SV, Serial Number 0137000001 X-ray equipment. Preventive maintenance shall include annual safety and performance inspection, and radiation leak survey/certification. Please include a copy of the purposed PMI service plan with your bid. B. Contractor shall inspect all electrical components and report any potentially hazardous conditions to the Agency’s Representatives. C. Contractor shall perform on-call emergency and non-emergency services within a 4-hour response time. D. Contractor shall thoroughly inspect and safety test all mechanical parts. E. Contractor shall provide Agency’s Representative a written report of services within five (5) days of service. Any repairs needed beyond the Scope of Work shall be indicated on report with an attached parts listing and the vendors hourly labor rates. F. Contractor shall provide required services by appointment between the hours of 8:00 a.m. and 3:00 p.m., Monday through Friday, state holidays excluded. Contractor shall establish and maintain documentation on the equipment to assure compliance with the preventive maintenance and corrective maintenance procedures established herein and Contractor’s Safety and Performance Testing Procedures (return with unsealed bid). This documentation shall be kept on file at some location mutually agreed upon by Contractor and Agency and shall include date and nature of work performed. H. All work which does not meet the Agency’s approval shall be corrected within ten (10) days. Agency reserves the right to withhold payment until such corrections are made. I. Damages to equipment caused by the Contractor’s negligence shall be repaired at no cost to the Agency.
