Phase 1: Project Initiation (Month 1-2) • Conduct a project kick-off meeting with key stakeholders. • Develop a project timeline and work plan. • Identify internal and external stakeholders to participate in the process. • Review and assess existing HPHD documentation, reports, and previous plans. Phase 2: Organizational Readiness Assessment (Month 3-4) • Conduct an organizational readiness assessment using NACCHO’s guidance. • Identify strengths, weaknesses, opportunities, and threats (SWOT) related to the HPHD’s current capacity. • Facilitate readiness discussions with leadership and staff. • Deliver a readiness assessment report with key findings and recommendations. Phase 3: Stakeholder Engagement (Month 4-5) • Develop a stakeholder engagement plan. • Facilitate focus groups, community forums, and interviews with key stakeholders, including HPHD leadership, staff, community partners, and residents. • Utilize qualitative and quantitative methods to collect stakeholder input. • Document and synthesize stakeholder feedback into a comprehensive engagement report. Phase 4: Environmental Scan (Month 5-6) • Conduct an environmental scan to assess internal and external factors impacting the HPHD. • Collect and analyze data on community health status, trends, policy changes, and other relevant factors. • Conduct a comparative analysis with regional, state, and national public health trends. • Produce an environmental scan report summarizing key findings, implications, and recommendations. Phase 5: Incorporation of Mission, Vision, and Values (Month 6-7) • Review the HPHD’s established mission, vision, and values statements. • Ensure alignment with PHAB accreditation requirements. • Facilitate discussions with stakeholders to ensure the mission, vision, and values are clearly reflected in the Strategic Plan. • Document how the mission, vision, and values are incorporated into the Strategic Plan. Phase 6: Strategic Priority and Goal Setting (Month 7-8) • Facilitate workshops with HPHD leadership, staff, and community partners to identify and prioritize strategic priorities and organizational goals. • Use data from the environmental scan and stakeholder engagement to inform goal setting. • Clearly define strategic priorities, long-term goals, and specific objectives.
