Specifications include, but are not limited to: Contractor shall provide SBVCC, as part of its charges for the services, such items, devises or supplies for the collection, storage and transportation of specimens and test requisitions forms. Contractor shall provide regularly scheduled courier services twice per day for the transportation of specimens from SBVCC to Contractor. Contractor shall make reasonable efforts to deliver or transmit results of a STAT nature back to SBVCC within 4 hours. Contractor shall make reasonable efforts to deliver or transmit results of a routine nature (general routine chemistries) back to SBVCC within twenty-four (24) hours. Contractor shall make reasonable efforts to deliver or transmit results of tests performed on specimens of a special nature (special chemistries, tissues, etc.) back to SBVCC within forty-eight (48) hours. Contractor shall provide staff during normal working hours to consult via telephone with SBVCC regarding test results. Contractor’s testing facilities shall be and remain duly licensed clinical laboratories under applicable federal, state and local law. Reasonable documentation of such credentials shall be provided upon written request. Contractor shall provide a designated sales representative to assist with new test information, technical updates and customer service issues. Contractor will provide SBVCC a written test report that includes results of the test, normal reference ranges for the test and comments deemed necessary by Contractor. Contractor will provide a phlebotomist to the facility three days per week to draw for routine scheduled labs (M-W-F). Each party shall maintain records for patients in such form and for such duration as may be required by Federal, State or local statues and regulations, and to make available to the Department of Health and Human Services, the U.S. Comptroller General and their designees upon reasonable request and in a reasonable manner its books, documents, and records relating to its provision of services procured through this Quick Quote as may be required by applicable statutes and regulations. Contractor agrees that these agencies and their designees have the right to audit, evaluate or inspect Contractor’s (and/or its subcontractor’s) books, contracts, medical records, patient care documentation, and other records, related to receipts of federal and state funds.
