The Contractor must perform all work and deliverables as follows: 1. Evaluations and assessments for clients referred by DHS must occur during regular business hours (8:00am – 5:00pm) from Monday through Friday. Occasional evening or weekend evaluations and assessments may be required. 2. For some referrals, coordination with the Project Officer or other County staff will be required on the date, time, and location of evaluations and assessments. 3. In-person, face-to-face assessments are preferred in all cases and are required for any guardian/conservator cases. Virtual assessments may occur if agreed upon with the Project Officer, and only if the nature of the required testing allows. 4. The Contractor must contact the County to schedule the initial consultation process within two (2) business days of when the referral is submitted to the Contractor. The evaluation or assessment must take place within 20 business days of the initial consultation. 5. The Contractor must consult with the County case manager, as well as other healthcare professionals, primary healthcare providers, medical records, family members, a valid substitute decision maker (power of attorney), a legal guardian and/or guardian ad litem on relevant background information regarding the individual’s ability to receive and evaluate information in order to make decisions...
