Contractor shall provide all shop, qualified labor, materials, equipment, and supervision sufficient to Operate/Administer State-Inspected Potable Water Supply Plant Operation and Monitoring Service in accordance with all Federal, State and local requirements. Contractor shall be required to Oversee and Administer Operational Services for the Potable Water System, including but not limited to the following: A. Requirements: 1. Check System (minimum) three times a week to ensure chemical feed systems are operating properly; 2. Prepare and submit on behalf of the Virginia State Police (VSP), all Virginia Department of Health (VDH) mandated reporting requirements for this site. Contractor to copy VSP Contract Administrator (CA) at Jason.a.crowder@vsp.virginia.gov of all reported records. 3. Provide System Operation of existing Potable Water Plant System Components in accordance with Manufacturer’s Recommendations and the requirements of the VDH Office of Drinking Water. 4. Order, collect and submit all required samples for testing to an independent testing laboratory. Contractor shall not utilize their own facility for testing. 5. Provide contractor owned/leased testing equipment and associated containers, as needed, to comply with VDH sampling and submission criteria. 6. Provide all reporting documents/logs to be maintained at site. Ensure that these documents are protected (3-ring binder) and readily available for review by VDH and VSP. 7. Contractor shall furnish all NSF Approved chemicals for duration of contract and/or required for system operation. 8. Contractor shall be responsible for coordinating/receiving any contractor shipment of supplies at site/location. 9. Maintain Water Supply Plant Facility/Room in clean condition acceptable to the Contract Administrator (CA). 10. Any Work performed must not interfere with on- going operations of the Facility. 11. Contractor shall include all associated costs of the scope of work (ie. transportation, fuel, overhead/burden, trip charges, mailing, FED-EX/UPS charges, Laboratory Fees, sample containers, labeling, labor and administrative costs) in Monthly Rate on Pricing Schedule. 12. Contractor shall be required to schedule visits with Contract Administrator (CA). Contractor will sign in/out at each visit for verification of visits per the contract. B. Equipment Repair Services/Additional Testing: 1. Contractor will not be required to execute equipment repairs; however, the Contractor shall make recommendations to VSP concerning any needed repairs or replacement of installed equipment; Provide technical/operational advice and guidance to VSP on as needed basis; 2. In the event the Contractor has the licensed ability to replace or fix the equipment, the Contractor shall provide a written quote of selected model and type with detailed description for review and consideration by the Contract Administrator. Contractor shall perform any additional testing that may be required by State and Local Government. 3. The Agency reserves the right to solicit other sources for any replacement or additional equipment as deemed in the best interest of the Commonwealth of Virginia. C. Contractor Qualifications: 1. Contractor /System Operator must have at a minimum DPOR State Waterworks Operator “ Class “ 4 ” License; 2. The awarded Contractor/and Operator’s shall be required to pass a Criminal History Name Search Request background investigation and fingerprinting (at no cost to the Contractor) prior to being assigned to this work. See attached Procedure and Authorization Form, which will need to be completed and notarized upon award of contract.
