The Puller Veterans Care Center is seeking a single Contractor (Offeror) to serve as medical director and provide in facility primary care medical and psychiatry services to the patients of the Puller Veterans Care Center. Medical Director Services: The Medical Director shall: Be responsible for implementation of resident care policies and the coordination of medical care in the facility. Be knowledgeable of state and federal regulations regarding skilled long-term care facilities. Meet all regulatory requirements for Medical Director functions within the facility. Ensure the appropriateness and quality of medical care and medically related care. Hold a current valid, unrestricted license in Virginia to practice medicine. Oversee (and help coordinate) the medical care and other designated care services. Be available during Federal and State Inspections to respond to surveyor questions. Be available to the Administrator and Director of Nursing for consultation in developing policies and procedures, review implementation of current policies and procedures regarding clinical care of residents to help ensure the policies and procedures are clinically valid and consistent with current standards of care. Update/review resident information in facility EMR (Electronic Medical Record) software in a timely manner. Be available to come in, as needed, to assist the Director of Nursing with resident clinical issues and/or concerns. Provide input to the Administrator and Director of Nursing regarding scope and services provided by the facility. Assist in the development of educational programs for facility staff and other professionals and provide training as needed to the facility staff and other professionals. Be knowledgeable about current professional standards of practice in caring for long-term care residents, to include Center for Medicare & Medicaid Services (CMS) regulations. Be available to meet with, or discuss with, other medical providers or physicians regarding timely response to facility needs, making visits, clinical issues, providing guidance and feedback regarding practitioner performance as needed, discussing and intervening (as appropriate) with a health care practitioner regarding medical care that is not consistent with current standards of care. Assist with developing systems to monitor performance of healthcare practitioners including mechanisms for communicating and resolving issues (also ensuring other licensed practitioners who may perform physician-delegated tasks act within the regulatory requirements and within the scope of practice as defined by state law). Be available to meet with families as needed to discuss the needs for their family member. Be available to fill in for attending physicians as needed to ensure appropriate resident care and compliance with regulations. Provide 10 to 15 hours per month on-site to perform medical director duties. As facility census grows, the Contractor must ensure the ability to supply sufficient MD/NP services in person, seven (7) days per week, to meet the clinical and regulatory needs of the Puller Veterans Care Center. The Contractor shall demonstrate staffing scalability and maintain coverage to ensure resident care is not disrupted. Be available 24/7 by phone to respond to medical necessities. Participate in the Facility Assessment and Quality Assessment and Assurance Committee and assist with issues related to the coordination of medical care/be available to assist with QAPI plans. Provide any written reports, as needed or requested by the Administrator or designee. Support and promote person-directed care (such as the formation of advanced directives, end-of-life care, etc.). Shall have a minimum of twelve (12) months’ prior experience as a Medical Director in a long-term skilled nursing facility. Must be available to begin duties on/or after November 1, 2025. Psychiatric Services The psychiatric services provider shall: Be responsible for implementation of resident care policies and the coordination of psychological care in the facility. Be knowledgeable of state and federal regulations regarding skilled long-term care facilities. Meet all regulatory requirements for facility psychiatric functions within the facility. Ensure the appropriateness and quality of psychiatric medical care and medically related care. Hold a current valid, unrestricted license in Virginia to practice psychiatric medicine and be a MD of Psychiatry. Oversee and help coordinate psychiatric medical care and other designated care services. Obtain consent from residents and/or their responsible parties prior to starting or changing any psychotropic medications. Be available during Federal and State Inspections to respond to surveyor questions. Be available to the Administrator and Director of Nursing for consultation in developing policies and procedures, review implementation of current policies and procedures regarding psychological/psychiatric care of residents to help ensure the policies and procedures are clinically valid and consistent with current standards of care. Be available to meet with families as needed to discuss the needs for their family member. Be available to fill in for attending psychiatrists as needed to ensure appropriate resident care and compliance with regulations. Provide 10 to 15 hours per month on-site to perform facility psychiatric duties. As the facility census grows, the Contractor must provide sufficient on-site psychologist, talk therapist, and psychiatric services five (5) days per week to meet the clinical and regulatory needs of the Puller Veterans Care Center. The Contractor shall demonstrate staffing scalability and maintain coverage to ensure veteran mental health care is not disrupted. Must be available 24/7 by phone to respond to medical necessities. Participate in the Facility Assessment and Quality Assessment and Assurance Committee. Be available to assist with QAPI plans.
