The intent of this solicitation is to establish a contractual price agreement with a qualified/licensed Optometrist to provide an on-site optometry clinic for up to 11 patients (with option to see more on mutual agreement) per month ,on a mutually agreed schedule at Piedmont Geriatric Hospital hereafter referred to as “PGH” at 5001 E. Patrick Henry Hwy. Burkeville, VA 23901. B. Piedmont Geriatric Hospital, located in Burkeville, VA, is a 123-bed geropsychiatric hospital. It is the only Virginia state facility that exclusively treats elderly persons aged 65 and over who need inpatient treatment for mental illness. 2.1. The regular business hours are between 9:00a.m. and 5:00p.m. 2.2. PGH is located at 5001 E. Patrick Henry Highway, Burkeville, VA 23922 2.3 The Contract is to run from October 1st,2025-September 30, 2026 with four, optional one year renewals for four years.
