I. Provider Agency Responsibilities The contracted provider will provide the following general components for the NonCongregate Homeless Shelter: A. Program Management Utilize Housing First, low barrier; harm reduction approaches to the short-term provision of shelter and services. Provide Housing Focused Case Management to all clients with a primary goal of obtaining permanent housing. Maintain membership and active participant in the Southeastern Virginia Homeless Coalition (Continuum of Care – “CoC”). Utilize the CoC’s Homeless Management Information System (HMIS) according to the CoC’s applicable written standards for programmatic data collection and reporting. Utilize the CoC’s Coordinated Entry System (including use of the CoC’s vulnerability screening tool or one approved by the CoC). Services must be provided without regard to actual or perceived race, ethnicity, color, sex, sexual orientation, gender identity, religions, national origin, ancestry, age, disability, marital status, source of income, familiar status, genetic information or domestic or sexual violence victim status. The provider must accept placement of individuals at night and on the weekends and holidays. Vacancies should be filled immediately and within forty-eight (48) hours unless extenuating circumstances require more time. All exceptions must be documented explaining the delay and steps taken to make the bed/unit available as quickly as possible. Develop a proposed operational plan for the City’s approval. The proposed operations plan must include a description of the process used by the provider to solicit input from a representative group of clients to inform the design and implementation of the program. The provider shall provide the City a proposed implementation plan for approval within thirty (30) days of contract award. The plan shall include the Operations Manual, twenty-four (24) hour staffing plan, security plan, literature, and any other related operational materials being used by the provider. Ensure that all residents have access to three nutritious meals per day including at least one hot meal. Provide program activities, facility infrastructure, equipment, staffing and support that ensures that staff and clients are as physically and emotionally safe as possible. Design a supportive services approach incorporating, at a minimum, the following: Peer support activities related to daily living, well-being, spirituality, leisure, and social integration Mental health care Substance use services Physical health care Benefits counseling and advocacy (i.e. SSI/SSDI, Veterans' benefits, unemployment, etc.) Support securing necessary transportation and/or providing transportation for required appointments, activities of daily living, and social integration Support and provide assistance with locating and acquiring permanent housing Support and provide assistance with seeking and gaining employment, as appropriate
