I. Provider Agency Responsibilities The contracted provider will provide the following general components for the NonCongregate Homeless Shelter: A. Program Management Utilize Housing First, low barrier; harm reduction approaches to the short-term provision of shelter and services. Provide Housing Focused Case Management to all clients with a primary goal of obtaining permanent housing. Maintain membership and active participant in the Southeastern Virginia Homeless Coalition (Continuum of Care – “CoC”). Utilize the CoC’s Homeless Management Information System (HMIS) according to the CoC’s applicable written standards for programmatic data collection and reporting. Utilize the CoC’s Coordinated Entry System (including use of the CoC’s vulnerability screening tool or one approved by the CoC). Services must be provided without regard to actual or perceived race, ethnicity, color, sex, sexual orientation, gender identity, religions, national origin, ancestry, age, disability, marital status, source of income, familiar status, genetic information or domestic or sexual violence victim status. The provider must accept placement of individuals at night and on the weekends and holidays. Vacancies should be filled immediately and within forty-eight (48) hours unless extenuating circumstances require more time
