Specifications include, but are not limited to: a. Provide copies of credentials and current licenses to practice in Virginia. b. Submit necessary information and allow for fingerprinting of all therapists providing services so a background investigation may be completed. c. Provide preventative and general medical care that promotes and enhances physical and emotional well-being, including evaluation, diagnosis, and treatment. d. Follow all prescribed physicians’ orders for therapies. e. Participate with the consumer’s team to ensure that a complete assessment in the areas of PT, OT, and Speech are completed and is thoroughly considered and addressed by the team as it develops the individual program plan before admission, after the first 30 days of admission and at least annually thereafter. f. Participate with the consumer’s team to incorporate the strengths, needs and care of PT, OT and Speech into the development and reviews of the individual program plan. g. Complete all required documentation of services provided including a summary of each appointment with findings, services provided and follow up recommendations. All comprehensive reports shall be submitted at least 10 days prior to the annual and quarterly meetings. h. Work collaboratively with staff and other service providers involved in the treatment of each consumer to ensure all needs are met. i. Provide flexibility with arranging appointments with consumers. j. Comply with all local, state, and federal regulations for ICF-IID provrams. k. Work with the consumer’s team to ensure that any restrictive techniques proposed are used only when warranted and with the required safeguards and approvals. l. Release the records and final recommendations for future care when the consumer is discharged or discontinues services with this provider. m. Submit monthly invoices for all services provided under this agreement.
